ObjectiveWe examined whether orlistat is associated with an increased risk of developing cardiovascular events (CV events) in subjects who use a clinically-prescribed lipase inhibitor. Methods: This observational study included 615 subjects with a mean age of 35 years who had a routine CV event in the Framingham Off-label Database between 1996 and 2017. Participants were defined as subjects who had used a clinically-prescribed lipase inhibitor (e.g., diet or exercise) or who had used no other known cardiovascular risk factor for at least 2 years before the index event. Cox proportional hazard models were used to assess whether orlistat was associated with a higher risk of CV events in subjects who used this medication. Results: Using the Cox proportional hazard model, a relative risk of cardiovascular death for those using orlistat was reduced to 0.7. Of the 615 subjects, 18.8% had a relative risk of CV events, and of those 18.8% were found to have a higher risk. Of the 615 subjects who had a relative risk of a CV event, 19.1% had a relative risk of an adverse event. Conclusion: Orlistat was not associated with a higher risk of developing CV events in subjects who used a clinically-prescribed lipase inhibitor. The overall conclusions of this study should be interpreted with caution due to the large number of subjects with cardiovascular events.

There were no differences in the rate of death between groups (hazard ratio: 0.9, 95% CI: 0.3–1.5).ResultsA high relative risk of cardiovascular event for subjects using a clinically-prescribed lipase inhibitor was observed in the Cox proportional hazard model, suggesting an increased risk of cardiovascular disease (CV event) for subjects who used a clinically-prescribed lipase inhibitor compared with those using no other known cardiovascular risk factor (hazard ratio: 1.4, 95% CI: 1.1–1.7). In addition, a relative risk of CV event for subjects using orlistat was reduced to 1.4, 95% CI: 1.0–1.7. The relative risk for a CV event was reduced to 1.2, 95% CI: 0.7–1.5 in the Cox proportional hazard model. These results suggest that orlistat was a risk factor for developing CV events in subjects who used a clinically-prescribed lipase inhibitor. The overall conclusions of this study should be interpreted with caution due to the large number of subjects with cardiovascular disease.
